Kaiser Permanente’s Life Care Planning is a tool to help you prepare for life’s “what ifs”.
Life care planning involves two major choices:
  • Selecting someone you trust to make your health care decisions if you are incapable.
  • Giving specific instructions about your care in the event of a sudden injury or illness.
These are very important decisions, so it is best to make them after carefully considering your values, beliefs and experiences.
We highly recommend that you have a completed advance directive and once you submit it, you may earn 10 Wellness University credits towards great rewards. You can complete an Advance Health Care Directive online or pick one up at any Health Education Center in South Sacramento, Elk Grove and Promenade.
