April 16th is NATIONAL HEALTHCARE DECISIONS DAY and we would like to invite you to “meet and complete” Advance Directive session on Tuesday, April 15, 12:30 p.m.

Meet the new Kaiser Permanente Advance Directive and complete your Life Care Planning. In recognition of National HealthCare Decisions Day (NHCD) on April 16th, the Regional Life Care Planning Team will be hosting two workshops for employees and physicians to help with planning and completion of the Advance Health Care Directive.
